This Commission was appointed on 29.10.1964 by H.E.The Governor-General, to inquire and report on the causes to the derailment and the measures that should be taken to prevent a recurrence of accidents. The Report was published as a Sessional Paper -No.V of 1965. The worst train Disaster in Ceylon since 1928 occurred on 17.10.1964 at Mirigama. Twenty-Five persons died, One hundred and forty were injured, among them seventy five were seriously injured.
Report on the following matters,
A. The causes that led to the derailment of the down Train No 400 on the Main Line of The Ceylon Government Railway between Ambepussa and Mirigarna on the morning of Saturday 17.10.1964.
B. The measures that should be taken to prevent recurrence of such accidents and to make recommendations in respect of tile above matters.
